By booking an appointment with Dr. Wesley Buckle, ND, you acknowledge and agree to the following:

1. Cancellation Policy

A minimum of 48 hours’ notice is required to cancel or reschedule appointments.
Cancellations or reschedules made with less than 48 hours’ notice may be subject to a $100 fee.

2. Residency & Location Requirement

Patients must be residents of California or Oregon, or be physically located in either California or Oregon at the time of their scheduled telemedicine visit.
By booking, you confirm that you meet this requirement.

3. Monthly Weight Loss Programs (Tirzepatide & Semaglutide)

These programs are structured as monthly treatment plans.
Follow-up visits are required every four weeks to continue treatment.
Missing a scheduled monthly visit may result in an interruption of your treatment plan, including prescription refills.

4. Payment Policy

Payment is due at the time of booking unless otherwise arranged.
Patients enrolling in monthly programs acknowledge responsibility for timely payments to ensure uninterrupted treatment.

5. Medical Limitations

Telemedicine visits are limited to conditions that can be safely managed remotely. In some cases, in-person care or referral to another provider may be required.
Prescriptions, including controlled substances, are provided only when medically appropriate and at the discretion of Dr. Buckle.

6. Communication & Records

Appointment reminders and important updates are sent via email or text. It is the patient’s responsibility to keep contact information current and to check communications regularly.
Patients are responsible for providing accurate health history and notifying the office of any changes in health status, medications, or care from other providers.

7. Consent to Treatment

By booking, you consent to receive naturopathic and telemedicine care from Dr. Buckle and acknowledge that outcomes are not guaranteed.
